The desired output format seems entirely unreasonable to me:
I don't see any plausible scenario where anybody would want that, particularly 3).
Can you give us an explanation why you chose this output format?
Also: Can a street appear twice in the same city, but with different (or even identical?) numbers? If so, what should the output look like in this case?
