There are several passing solutions that only modify the last two letters, so that would be impossible.

Prehaps there was a specific word (test case) that was confusing?

• The newest edit did add that section:

If you recive anything that is nested, you should separate them if the quote character is the same, and include them in the outermost quotation if they are different...

• Following those rules:

w"i'e"r' -> { "i'e" }

• Yes, "So 'is' 'this'" -> { "is", "this'" } would be correct.
